Our chapter welcomes you to join our cause, which is to inspire hope in patients with heart disease. You must be a chapter member to participate in the Mended Hearts Visiting Program. Our meetings and events are open to all heart disease patients and guests, including those who have a genetic predisposition to heart disease.

Complete the form and mail it with a check for the first year's dues to the Chapter Treasurer whose name and address appears on the bottom of the Application Form.
